Perioral Dermatitis: Symptoms, Treatment, and Prevention
Red bumps around your mouth or nose? Learn the signs, causes, and safe treatments for perioral dermatitis—and how to prevent future flare-ups.
Retinol, a derivative of vitamin A, has long been heralded as a gold standard in skincare, celebrated for its remarkable ability to address acne, reduce wrinkles, and even out skin tone. However, for many who introduce retinol into their routine, an unexpected challenge may arise: an apparent increase in breakouts. This phenomenon often leads to a pressing question—can retinol actually make acne worse, or is it simply part of a process known as skin purging?
Retinol’s efficacy is rooted in its ability to accelerate cell turnover. By promoting the rapid shedding of old skin cells and encouraging the growth of fresh ones, retinol helps unclog pores, reduce the appearance of fine lines, and diminish hyperpigmentation.
It is prescribed both over-the-counter and in stronger forms (retinoids) for conditions ranging from moderate acne to signs of aging. While its benefits are substantial, retinol is also renowned for causing initial irritation, dryness, and, in some cases, breakouts.

Skin purging is a temporary reaction that may occur when starting certain active skincare ingredients, primarily those that accelerate skin cell turnover, like retinol, alpha hydroxy acids (AHAs), or beta hydroxy acids (BHAs).
The process involves bringing existing microcomedones (the precursors to pimples) to the skin’s surface at an accelerated rate. In simpler terms, retinol speeds up the life cycle of pimples lurking beneath the surface, causing them to come to a head—and then heal—faster than they would have naturally.
It’s essential to distinguish between purging and regular breakouts. Skin purging typically presents as small whiteheads, blackheads, or pustules in areas where you commonly break out.
These symptoms are transient and may resolve within four to eight weeks as your skin adjusts and clears out underlying congestion. On the other hand, if you develop new, persistent breakouts in areas where you’re not typically prone to acne, or if symptoms last longer than a couple of months, you might be having a sensitivity reaction or simply using a product that isn’t right for your skin type.
Unlike inflammatory acne that can be triggered by comedogenic (pore-clogging) products, retinol’s effect is more about timing than causation. Retinol doesn’t create new blemishes; rather, it expedites the emergence of existing ones below the skin’s surface. The purging phase, while uncomfortable, is generally indicative of the product doing its intended job.
For most people, skin purging induced by retinol lasts between two to eight weeks. The duration may vary based on the strength of the retinol, the frequency of use, and individual skin sensitivity. It’s important to continue using the product as directed during this time unless severe irritation occurs, in which case a dermatologist should be consulted.
Gradual introduction—using retinol every other night or in lower concentrations initially—can help mitigate the intensity of purging.
While frustrating, skin purging is a temporary setback on the path to clearer skin. If you’re experiencing purging, patience is key. Most users find significant improvement in their complexion once the purging phase subsides.
However, if breakouts are severe, accompanied by swelling or pain, or lead to prolonged redness and sensitivity, it’s wise to consult a skincare professional to rule out adverse reactions.
Retinol is a powerhouse ingredient with proven results, but its transformative journey often comes with some turbulence in the form of skin purging. Recognizing the difference between a standard breakout and a purge can provide reassurance and help you stay the course.
With careful management, most individuals can navigate through this challenging phase and eventually enjoy smoother, clearer, and healthier-looking skin. If in doubt, always seek guidance from a dermatologist or skincare expert to tailor the perfect retinol routine for your unique skin needs.
